Guidelines on Requesting Medical Records

Transactions Hours and Schedule of Release

The Medical Records office accepts requests for Medical Records from Mondays to Fridays, 8:15 a.m. until 5 pm.

Patients can request the following records with the corresponding schedule of release:

CERTIFICATION

Certificate of Records/Materials within the day of request
Certificate of No Records/Materials within the day of request
Certificate of Confinement (CC) within the day of request
Medical Certificate (MC) within the day of request, If available
Medico Legal Certificate within the day of request, If available
Birth Certificate 1 week upon Registration
Death Certificate within the day of request, if available
Fetal Death Certificate within the day of request, if available

DIAGNOSIS RESULTS

Laboratory results within the day of request
Examinations

(X-ray, UTZ, CT scan, 2D Echo, ECG, Biopsy, OR Tech)

within the day of request, If available

 CLINICAL SUMMARY

Discharge Summary (DS) within 3 working days after the date of request
Clinical Abstract (CA) within 3 working days after the date of request

How to Request:

    • Submit Accomplished form to the Medical Records Office
    • Request forms may be secured form the Medical Records Office
    • Reques through Telephone will not be accomodated

Claiming Requested Results:

    • If the one who will claim the results is the Patient himself/herself, he/she should present an ID.
    • If Authorized Representative will get the request, an Authorization Letter signed by the Patient, photocopy of Patient’s ID, and the Claimer’s ID is needed.
    • A Charge Slip will be issued by the Medical Records Staff.
    • The Record shall be issued upon presentation of the Official Receipt.
    • For each Record Requested, Copies given are CERTIFIED TRUE COPY (Considered Original):

  • Under the Medical Records Guidelines you can be charged a reasonable fee for copying records.
